The foundation's work is aimed at protecting the rights and interests of people who use drugs. Since 2009, the fund has been implementing the Harm Reduction - Moscow project aimed at street social work with people who inject drugs in order to prevent HIV, hepatitis, and overdose. In addition, the organization provides medical and social assistance to citizens in need and also implements a "Family Project" aimed at helping children from families in which they use drugs.

The organization was partly funded by the Western sources - private foundations and the Global Fund to Fight AIDS, Tuberculosis and Malaria, however, it was not a problem during the inspections by the Ministry of Justice in 2013 and the Foundation was recognized as a "foreign agent" only in June 2016.
